As an independent provider, we serve individuals nationwide, including those in hard-to-reach communities, meeting them where they are most comfortable: in their homes.

Identify & Contact

We reach out to eligible members or patients who meet the criteria for in-home health assessments.

Schedule the Visit

Our team coordinates with the patient to schedule a convenient time for a visit with one of our healthcare experts—either a nurse or a nurse practitioner, depending on the recommended assessments and care.

Assess & Coordinate

Our clinician conducts a comprehensive in-home health assessment, securely transmitting results to the patient's primary care provider and closing any gaps in care. This promotes better health outcomes while reducing costs associated with unaddressed healthcare needs.
